Information we collect
When you submit our Contact or Request Appointment forms, we collect the information you provide (name, contact details, insurance plan, reason for visit, preferred times). This website does NOT collect Protected Health Information (PHI). Please do not include medical history, diagnoses, prescriptions, or other PHI in form submissions.
How we use it
We use form submissions solely to respond to your inquiry and schedule appointments. Submissions are stored securely and are not shared with third parties for marketing. We use service providers (for example, email delivery and hosting) to operate this website under contracts that limit their use of your information.
Cookies and analytics
We use Google Analytics, a third-party analytics service provided by Google LLC, to measure aggregate site usage such as which pages are visited and how visitors reach our site. Google Analytics sets cookies and collects device and usage information — including your IP address and an approximate location derived from it — and processes that information on Google's behalf, subject to Google's privacy policy. We have not enabled Google Signals or advertising personalization. We do not use third-party advertising cookies, and we do not send the information you enter into our Contact or Request Appointment forms to Google — those submissions go only to the practice.
Your choices.You can opt out of Google Analytics using Google's browser add-on at tools.google.com/dlpage/gaoptout, or by using your browser's cookie controls. We honor Global Privacy Control (GPC) browser signals. California residents may also exercise rights to know, delete, correct, and opt out of the sale or sharing of personal information by contacting us using the information below. We do not sell personal information, and we do not share it for cross-context behavioral advertising.
SMS / text messaging
This section explains how we collect, use, and share information when you opt in to receive SMS text messages from Frontline Healthcare Endocrinology.
How we obtain consent. We send SMS text messages only to individuals who have opted in. You provide consent by checking the SMS opt-in box on our appointment request or contact form, or by giving your mobile number and consent to our staff at the practice. Consent is voluntary and is not a condition of receiving care.
Information we collect. When you opt in to receive SMS messages, we collect your phone number and your consent to send you SMS messages.
How we use it. We use your information to send the SMS messages you have opted in to receive — appointment reminders, practice updates, account and records notifications, and other customer-care messages. We do not send marketing or promotional text messages.
Sharing. We do not share your phone number or SMS opt-in information with third parties for marketing purposes. Mobile opt-in data is never sold or shared with anyone for any purpose.
Your rights. You can opt out of receiving SMS messages at any time by replying STOP to any message we send you. Reply HELP for help. Message and data rates may apply, and message frequency varies.
